HDI agrees to provide shelter, feeding, exercise, play and living environment as explained on the HDI web site (www.thehappydoginn.com). Owner acknowledges reading the web site description under “Rates, Hours and Things to Know” tab, and acknowledges inspection of HDI and that the conditions are acceptable. Special care instructions shall be provided by Owner in writing.

Fees, Charges:

Owner agrees to pay $70.00 for a single pet, and $40.00 for each additional pet from the same home in the same suite per suite per day. One half these amounts for ½ days. A half day charge applies for afternoon pick ups. Daycare for a full day is one half of daily rate and one 2 hour playgroup is one half of that.

Owner shall make full payment for boarding services in the event of early pick up of Pet(s). Owner agrees to pay a 3 day fee if cancellation is less then 3 complete days prior to scheduled arrival. (Exception; Christmas and Thanksgiving Holidays are confirmed 30 days in advance and are non-refundable). Stays may be extended in event of Owner emergency, but only if HDI agrees and has space available. If pick-up is on a date later than agreed, Owner agrees to pay for all days at the rates stated. For all fees and charges incurred under this agreement, Owner authorizes HDI to charge Owner’s credit card upon the date agreed for pick-up and thereafter as incurred. Rates are subject to change, Owner will be advised of any change prior to drop off. Pets will not be released to anyone other then those listed above without prior written consent. Owner will be responsible for all legal fees incurred by HDI in its effort to enforce this agreement.

Food, Medicine:

Owner agrees to provide prepackaged food and medicine for each day of Boarding, plus two extra days, as well as any medication instructions at time of drop off. All dogs are required to be current on Rabies, DHPP and Bordetella vaccinations. During flea and tick season, pet(s) will be treated with a flea and tick control product if they are not currently on a program. There is a onetime charge per stay of $10.00 per dog.

For security reasons, DO NOT enter your Credit Card number. When this form is received, we will call for Credit Card information. Please add name, expiration and code. Card number is kept separately.

If an emergency does arise requiring veterinary care for Pet, care will be provided by HDI’s choice of veterinary hospital. Owner authorizes the maximum amount to be charged on Owner’s credit card for medical care if owner or contacts are not reachable: *

HDI will not authorize medical services above the amount indicated above, { EX: if you circle $1,000.00 and it takes $1100.00 to help your dog, we will decline medical care}. Owner agrees to then take over all arrangements for care and payment with the hospital.

Minor medical problems as determined by HDI, that do not require veterinary care, will wait for owners return. It is owners responsibility to notify HDI if there is pet insurance.

Play Group Authorization and Release:

Owner would like the following Pet(s) to be allowed in playgroups. Owner understands and acknowledges that there is always the possibility of injury to Pet either to itself or by another dog, and that dogs may be unpredictable. Owner hereby releases from any and all liability HDI (and all of its care providers, owners, officers and employees) for any injury to Pets associated with being in a playgroup.

Limitation of HDI:

The maximum liability of HDI for any failure to perform here under or for any harm, other than intentional and willful, to Pet(s) for any situation shall be the cost of the stay during which the harm may have occurred, but not to exceed $500.00. There shall be no liability for acts of nature, ingestion of foreign objects, insect infestation or bites, contact with wildlife, lightening or other Acts of God. Abandonment of Pet: The provisions of Arizona Revised Statues 3-1310. shall apply.
